Contact the practice online

You can contact the practice online by filling in one of our contact forms on our online access page. There are separate forms for contacting a doctor, a nurse, asking for a fit note or requesting an online prescription. If you would like to contact the practice about any other matter, then please follow the link for contacting reception. 

There are many reasons why you might want to contact reception, for instance, to tell us about a change in your circumstances or contact details, to make enquiries about your health record or let us know that you have stopped smoking. You can also use this form to give feedback, make a suggestion or make a complaint.

Making a complaint or suggestion

We take complaints very seriously, and would like to be able to deal with any complaints swiftly and to everyone's satisfaction whenever possible. We have an in-house complaints procedure.  If you have a complaint, you should contact the practice manager either through the practice in the usual way, or by filling in the reception contact form online. You can download our complaints forms which are below, or ask for a copy of these at reception.  It is always very helpful if you can make any formal complaint in writing, although this is not essential and we will still take a verbal complaint just as seriously. Your complaint will be fully investigated and dealt with promptly.

We are always happy to receive suggestions as to how we could improve our service, and we greatly appreciate the positive feedback that so many of our patients kindly give to us - it really does help keep up the morale of all our staff to know when we have done things well.

We are located on Binscombe Lane, on the edge of Farncombe in Godalming

Telephone lines are closed from 1-2pm and 6pm-6.30pm, but urgent calls can still be recieved.
