Technical Issue with Online Forms - 05.05.2026

On Tuesday 5 May, we experienced a temporary issue with our online forms. This was due to a problem with a third-party provider and was outside of our control.
Between 8:00am and 8:15am, patients were able to complete forms via our website. However, these submissions were not received in our inbox. Although the forms did not reach the final submission confirmation page, this may not have been apparent to patients who do not use the system regularly.
If you believe you submitted a form during this time, please resubmit it using the links available on our homepage.
The forms were taken offline at 8:15am. The issue was resolved by the third-party provider at 8:50am, and the service was restored shortly afterwards.
